The Iron Balance of High Dose Oral Iron Supplement with SC rHuEpo in CAPD patients

Abstract

|
Adequate iron supplementations are crucial to optimal response to rHuEpo therapy. But the iron indices; ferritin, discriminant function (EF) score could not represent the need for available iron in patients on rHuEpo. Net projected iron stores
(NPIS)
seems to be for new hemoglobin synthesis.
We compared the iron indicies in 9 CAPD patients with initial net projected iron dificit before and after thrice a week SC rHuEpo administration of 149.7 U/Kg/week. The daily oral dose of 320 mg of Fe+++ are administered until the target Hct of
30%
is
achieved. The mean interval for target Hct was 8.5 weeks.
We suggest that 1) Initial ferritin and DF score could overestimate the iron stores in CAPD patients on rHuEpo. 2) NPIS may be more reliable index for iron supplementation in CAPD patients on rHuEpo. 3) Daily high dose oral oron supplementation
could
maintain iron balance during rapid red cell production in patients with initial net projected iron dificit.
